It is our goal to provide quality dental care at a reasonable cost.  Payment for dental treatment rendered is expected at the time of your appointment.  We accept VISA, Master Card, American Express and Discover. We also offer a 0% financing option.

You will be provided with a written Treatment Plan at the end of your examination appointment estimating the cost of your needed dental treatment.  Financial arrangements can be discussed at this time.

Insurance

If you have dental insurance, please bring your insurance plan information to your appointment.  Dr. Clay has an insurance friendly practice.  At the end of each visit, we will prepare a complete insurance form for you to submit to you insurance company.  This form is recommended by the American Dental Association and universally accepted by dental insurance companies.  This form will be completed for you and ready to mail to your insurance company.

If documentation, such as x-rays, is required with your insurance claim, we will submit the Insurance Claim form for you at the end of your appointment and provide you with a copy for your records.

For the patient with dental insurance who wants to avoid out of pocket expense for insured dental benefits; we have found these patients will charge their dental treatment to their credit card and by the time the card statement is received, they should be reimburse by their insurance company.
